Ce suivi individualis\u00e9 permet aux professionnels d\u2019intervenir de mani\u00e8re proactive et cibl\u00e9e, assurant la <strong>meilleure prise en charge<\/strong> possible. Ces donn\u00e9es en temps r\u00e9els cr\u00e9ent un <strong>mod\u00e8le de pr\u00e9diction des r\u00e9sultats m\u00e9dicaux toujours plus pr\u00e9cis.<\/strong> Le patient y joue un r\u00f4le clef, ayant une voix directe dans leur suivi post op\u00e9ratoire, gr\u00e2ce \u00e0 l\u2019application Care4Today Monitor de Johnson &amp; Johnson.<\/p>\n<p>Le syst\u00e8me se base sur un algorithme d\u00e9cisionnel pour permettre par des questions rapides et simples, plusieurs fois par jour, de surveiller l\u2019\u00e9volution du patient, permettre une vision objective des processus de r\u00e9tablissement et activer des messages d\u2019alertes pour une intervention rapide si besoin. Les r\u00e9ponses des patients sont affich\u00e9es sur le tableau de bord des soignants et renforcent ainsi la communication permanente entre patients et soignants. Il s\u2019agit d\u2019un r\u00e9el outil de monitoring post op\u00e9ratoire et de communication qui propose aussi au patient des informations et un suivi de ses objectifs, son alimentation, etc.<\/p>\n<p>&nbsp;<\/p>\n<\/div>\n<div class=\"x11i5rnm xat24cr x1mh8g0r x1vvkbs xtlvy1s x126k92a\">\n<p><span style=\"color: #ff6600;\"><strong>Une accessibilit\u00e9 maximale<\/strong><\/span><\/p>\n<p>Le <strong>coordinateur digital<\/strong> est une nouvelle fonction professionnelle cr\u00e9\u00e9e et form\u00e9e sp\u00e9cifiquement pour r\u00e9pondre aux besoins du projet. C\u2019est <strong>la personne indispensable au lien patient-application pour \u00e9viter la fracture num\u00e9rique<\/strong> bien entendu et s\u2019assurer de la bonne compr\u00e9hension et utilisation de l\u2019application par le patient. Son r\u00f4le sera d\u2019informer et d\u2019accompagner le patient dans l\u2019installation et la mise en place de l\u2019application tout en assurant en permanence le relais entre le patient, le staff m\u00e9dical et l&#8217;application.
